SWIRCA & More has received certification to be a VA provider for the Veteran-Directed Home and Community Based Services program. This program provides funding for Veterans of all ages who are at risk of nursing facility placement to help pay for services and goods they need to remain living at home. A key characteristic of this funding is that is allows the Veteran to direct their own care and hire their own personal care aides (including family and friends). The funding can also be used for other services such as paying for home modifications like ramps or widening doorways to increase accessibility.
SWIRCA & More has trained Care Coordinators who will help the Veteran enroll in this new program, along with providing on-going support and assistance for the Veteran as they hire their own workers and direct their own care.
This services is available in all six of SWIRCA’s counties- Vanderburgh, Warrick, Posey, Perry, Gibson and Spencer.
How Does a Veteran enroll into the VD-HCBS Program?
- -The Veteran must have a primary care physician within the VA system; if they do not have a VA physician, they can call the Evansville VA Health Care Clinic at 812-465-6202 to being the registration process.
- -The Veteran must be at risk for nursing facility placement if they did not receive assistance with their daily living needs.
- – The referral must come from their VA physician or VA social worker.
