Pre-admission Screening applications are processed dependent upon the current location and situation of the individual.

Home screens:  For patients residing in their own home, group home or assisted living facility, an assessment is completed prior to admission unless an emergency situation exists.  Documents are submitted to SWIRCA & More and Options Counselor completes assessment.  Decision is given within 20 days.

Direct Admit from the Hospital:  For patients that have been admitted to the hospital, the Options Counselor will complete the assessment at the nursing home.  Exceptions exist.  See PASRR Level II information.

Out of State Screenings: Paperwork and decisions are completed prior to admission into an Indiana nursing facility.  The exception is an out of state resident admitted to an Indiana hospital after treatment in their emergency room.

Emergency nursing facility admissions;  For patients at home that need immediate placement,  the nursing facility completes an emergency intake form and will fax to SWIRCA & More.
