NATIONAL FAMILY CAREGIVER SUPPORT PROGRAM
TITLE 3E

The National Family Caregiver Support Program provides multi-faceted systems of support services for family caregivers, including access information and assistance; individual counseling, support groups and training, respite and supplemental services.

Services include:
Information to caregivers about available services
Assistance to caregivers in gaining access to the services
Individual counseling, organization of support groups and caregiver training to assist the caregiver in the areas of health, nutrition and financial literacy and in making decisions and solving problems relating to their caregiving roles.
Respite care to enable caregivers to be temporarily relieved from their caregiving responsibilities.
Supplemental services, on a limited basis, to compliment the care provided by caregivers.

NATIONAL FAMILY CAREGIVER ELIGIBILITY

Care recipient must be 60 years of age or older and requires assistance with two or more activities of daily living such as bathing, dressing, grooming or toileting.
OR
Be a grandparent or relative caregiver by blood, marriage or adoption of a disabled child younger than 18 years old.  Caregiver must live with the child and be the primary caregiver.  Caregiver must be 55 years of age or older and be the legal guardian of the child.
OR
Be a caregiver over 60 years of age for a care recipient that requires assistance in two or more activities of daily living.
