CHOICE ELIGIBILITY CRITERIA
Individuals must meet specified eligibility requirements in order to qualify for CHOICE services. These requirements are specified by the State of Indiana.
Individuals must be at risk of losing their independence which means the individual is unable to perform two or more activities of daily living as determined during the in home assessment performed by the SWIRCA & More Case Manager using the eligibility screen tool.
The individual cannot have more than $500,000 in assets documented via completion of the Asset Test Worksheet.
The individual must have a Medicaid determination within 90 days of CHOICE application being made.
Clients with income over 150% of Federal Poverty Guidelines or $1,371 per month may be charged a cost share depending on their amount of adjusted income by allowable deductions. Allowable deductions include monthly medical expenses, health insurance costs and home care costs as paid by the consumer. Case Manager will complete cost share worksheet during in home assessment.
CHOICE funds may only be used after all other available resources of funding for in home services have been accessed such as Medicare or Medicaid.
