HOME AND COMMUNITY BASED SERVICES
SWIRCA & More administers In-Home and Community Based Services for all age groups with a goal of increasing the client's independence and preventing premature nursing home placement while increasing the quality of life of the individual.
Funding for services include Medicaid Waivers, CHOICE, Social Services Block Grant (SSBG), Older Americans Act (Title III) and Family Caregiver in addition to funding raising dollars and donations.
Eligibility for subsidized services is based on a need for assistance, health impairments and risk of institutionalization. Some programs have age eligibility or income based requirements.
Funding for some programs may be limited or waiting lists may apply.
