CASE MANAGEMENT
A service comprised of assessment and related activities, included care planning, service linkage, advocacy, service monitoring, options counseling and reassessment. Case Managers are trained professionals who provide quality, person-centered services. They act on behalf of individuals by guiding them to services, agencies or programs to meet their social, emotional, physical and economic needs and to prevent premature nursing home placement. They conduct interviews in the privacy of the client's home. The goal of Case Management is to help clients maintain their independence.
